You will be reimbursed for the actual purchase price, excluding tax, delivery, and installation, on your dated sales receipt up to $3,000. You must provide a dated sales receipt and product serial number to be reimbursed. **Shopping spree is your choice of a $500 giftcard to Nike.com or Nordstrom.com.  Exchanges cannot be made. Official Contest Rules: Submissions are due by 12:00 A.M. (C.S.T.) on December 10th, 2008. By entering, all participants agree to be bound by the official rules. Prize winners will be selected on or about December 12, 2008 from among eligible entries received. Winners will be notified by mail, email, or phone. Prizes will be delivered to verified winners. The decision of the judge(s) is final. Limit one prize per family. Employees and immediate family members of Brunswick Corp. and Life Fitness (collectively "Sponsor"), are not eligible to win prizes. All applicable taxes on prizes are the sole responsibility of the winners. All prizes claimed will be awarded. If any prize goes unclaimed, there will be a random drawing to award such prize to an alternate winner. Prizes are non-transferable and no substitutions or cash redemptions will be made. Sponsor is not responsible for incorrect or inaccurate information. Each winner will be required to sign and return an affidavit of eligibility and a liability/ publicity release within 10 days of notification or prize will be forfeited. By accepting the prize, each winner agrees to release Sponsor, its partners, affiliates, officers, employees, representative and agents, including, without limitation, its advertising and promotion agencies, from any and all liability, loss, damage or injuries of any kind sustained in connection with their participation in the contest, promotion or from their acceptance, use, misuse of any prize. Each winner further acknowledges that said parties have neither made nor are in any manner responsible or liable for any warranty, representation or guarantee, express or implied, in fact or in law, relative to any prize, including, but not limited to, its quality or fitness for a particular purpose. Release For valuable consideration received, I hereby grant to Life Fitness a fully paid, irrevocable, royalty free, worldwide license to use and reuse, publish and republish, either in whole or in part, individually or in conjunction with any other work, my name, signature, image, likeness, story and voice in Life Fitness' copyrighted print advertising and in connection with the merchandising and promotion of equipment manufactured, sold and distributed by Life Fitness. I hereby release and discharge for myself, and my heirs, legal representatives, successors and assigns, Life Fitness and Brunswick Corporation from any and all claims and demands arising out of, or in connection with, the use of my name, signature, image, likeness, story and voice, including, without limitation, any and all claims of libel or invasion of privacy. By checking the box below, I, certify that (1) I am of legal age; (2) I have the right and authority to enter into this release; and (3) I have read the foregoing and understand the meaning of the same.
